Shelter Eligibility & Intake

Getting the Help You Need

Eligibility Requirements:

  • Must provide ID (AZ ID, Social Security Card, or Birth Certificate)
  • Must be legally and physically able to work
  • Couples will be housed separately
  • Respectful behavior is required in all shared spaces

Check-In Times:

Monday – Friday: 4:00 PM – 5:00 PM

First come, first served.

Getting Started

  • Complete Intake Form & Initial Questionnaire

    All clients are required to complete an intake form and initial questionnaire. This helps us better understand your situation and provide the most appropriate support.

  • Descriptive Action Plan (DAP)

    Following intake, we will work together to develop a personalized Descriptive Action Plan (DAP) that sets clear goals to support your progress toward stability and independence.

  • Partner Referrals

    We collaborate with trusted partner organizations to help you achieve the goals in your DAP. This may include:

    - Job readiness and placement programs

    - Transitional or permanent housing

    - Recovery and treatment services

    If we are not the best fit for your needs, we will do our best to refer you to someone who is.
